Flashing D

so I went to my monthly get together, and on the way home the engine light came on, great I thought, just what I need now, later I went out to the store, and on the way back the car was hesitating and D flashing along with one wrench flashing and one steady, I was gonna go to another store but fig I better get home, turned it off to see, it started but wouldn't go back in R or D

Code reader available? Are you close to someone that owns a scangauge? How old is your battery? Is it in top condition? The shifter motors have to have full electrical power or they become shiftless.
One of the upgrades that I did as a precaution was to upgrade the grounding connection from the battery to the frame. It appears a little on the spare side.

havnt changed the battery since I got it in 2011, last was checked a few months ago it was 12v, im close to a place that will check the codes
