Going long distances (50+miles) with over 20 smarts playing follow the leader does not work. I (and 20 of my closest smart-friends) know that from experience.

I'm really more interested in the legal side of this. The insights about Harley riders is interesting, but I still think maps, directions and a time table are the best way to go.

Perhaps if ever one in the caravan was given directions to the final place and checkpoints to meet at on the way, it would not really be a caravan of Smarts? Perhaps it would be more like a road rally? (just thinking out loud).

I have driven thousands of miles on antique car tours so I have a little experience on this subject.
A map is necessary. If on two lane roads spread out so that other traffic can pass. Many states have laws about delaying traffic. Multi-lane highways are less of a problem.
When you are continuing on a road there is no problem getting lost. One method used is when there is a turn, exit or change of some kind the second car in line pulls over and indicates the change and waits until the last car of the group passes and than follows. This works on country roads with little traffic and places to pull over. Another method is that you are responsible for the car behind you. If you turn or change direction just make sure the car behind you is aware. As long as everybody looks out for the car behind him nobody gets lost. Again the amount of traffic and places to pull over make difference.
Antique car tours usually take creative, scenic routes rather than go directly from one place to another so getting lost is more of an issue. Staying on a road is pretty easy you just need to know when there is a change. Just obey traffic laws, have respect for other traffic and have fun.
