I concur to us the cause of death is irrelevant to us. However to an accident investigator, it is very relevant, and might even bo to MB. To preclude other similar deaths there is a lot that can be learned and applied from this mishap, both from a design and metallurgical as well as from a human factors point of view. I speak from many years experience of picking up small parts and bits to determine cause and for prevention.
